Regular preventive care will keep your dog healthy and plays a vital role in the early detection of any potential health issues, illness, and disease. At Lincoln Park Veterinary Hospital, we recommend yearly wellness exams for adult dogs. As a member of the AAFP (American Association of Feline Practitioners), we strive to provide a high level of veterinary care for all cats.
I understand that all fees are due at time of service. If you are in need of alternative payment arrangements please discuss that prior to completion of services.* I hereby authorize the veterinarian to examine, prescribe for or treat the above described pet(s). I understand that all animals must be current on vaccinations and testing for boarding, grooming, daycare and hospital procedures.
In order to maintain our high standard of service, we require that payment is made at the time services are rendered. We provide estimates for all surgeries and hospitalizations. If you have not received an estimate, you are welcome to request one at any time for your expected charges. We accept cash, checks, and all major credit/debit cards.

Absolutely LOVE Dr Kelly! I think she is hands down the best Veterinarian I have ever had the pleasure to meet and I am so extremely grateful that my Gracie and I found her. She has a deep passion and love for her job, demonstrated through the gentle care she gives to all of her patients. As a healthcare professional myself I have seen many types of doctors. Dr. Kelly stands out as one of the elite in my book, she has it all, knowledge, compassion, and a genuine love and concern for the animals that she cares for.
My pet's will raise their ears and wag their tail if I say let's go see Dr Kelly! Thank you for the ultimate care you always give my Grace. I have gone here on and off (due to Covid and increase demand for vet care). Years ago I had my first visit with my dog Bella, an American bulldog, who had bad allergies that other vets couldn't control. I paid a fortune going to the vet every 2 weeks and watched my dog suffer. I was referred by a friend and 1 visit later my dogs issues were under control.
My recent experience yesterday left an ever bigger imprint on me. My 17 year old cat is not well so the staff worked hard to get me into cancellation spots, calling me several times determined to get him in. It was clean (which I do look at) and the doctor was extremely sensitive and realistic about options, even with my crying all over. I have been to almost every animal around.
The prices are on par with other vets in the area but the care is much much better. Only thing I might recommend is going over pricing before treatment. Thank you for your care.

Whenever i enter, i feel like im walking into a comfortable space and like im at a doctors office that really appreciates you being there. Staff are so understanding, sweet, and helpful. My senior dog suddenly had a couple of pretty big growths at his groin. I called and was able to get an appointment with Dr. Kelly the next day. The staff welcomed us and did a thorough history workup. Dr. Kelly did a great job of working with Rio, did some blood work and examination and was able to diagnose him with non-life-threatening fatty cysts. Rio and I were both very grateful!

I highly recommend them! They saved my cat Colby's life. Dr. Disek is very thorough and kept me informed throughout his hospital stay. I never met such a knowledgeable and informative vet! So glad I found them! Will be bringing my other cat there in the near future. The staff are all very friendly, compassionate and competent as well. Thank you from the bottom of my heart!

I haven't brought any of my animals here yet, but just talking to the girl over the phone i will start. I had a question about a feral cat I've been taking care of and she spent at least 10 mins explaining as to how to handle the situation. Very helpful and compassionate. Just adopted a dog and had to find a vet to take her to. Setting up an appt for her and getting in were easy. Couldn't afford to get everything she needed right away, so we set it up to get her all vaccinated and up to date in a few appts. Everyone was very kind. And they made my pupperoni feel safe too. We brought our cat Sabrina to have her euthanized when her time had come to depart from our family. this is the most caring, compassionate, and warm-hearted veterinarian clinic I have ever been to in my life. The staff are so friendly and respectful. They were extremely gentle and caring with Sabrina in her final minutes with us. The doctor was extremely professional and allowed us to spend our final moments with her before and after the euthanization process.
We were given a paw print which I will forever hold onto. I have nothing but pure respect for this clinic and it's staff. Very professional and very loving towards their patients and the families of pets. 2 weeks after Sabrina's passing, the clinic sent us a heartfelt card in the mail with the signatures and messages from their staff sending their condolences. I completely and fully recommend this clinic for all of your needs. They are good people, and I have the utmost respect for their staff. Thank you.

Dr. Bodenstein is honestly the nicest and most patient veterinarian I've ever met. Very caring and professional. I'm so glad we chose Lincoln Park veterinary hospital for our 2 cats. The nurses are very friendly, however, for them to be a vet clinic without prices online and then they charged me $200 just to bandage a wound. i thought my dog was going to need stitches. and they took him back before i could say i could do it myself. so they literally trapped me into their care and then when i told them i didn't have $200 on hand they proceeded to tell me they have no payment plan options. just go to the humane society or all about animals. it's a shame people are playing people that just want to care for their dogs. i hate this world
